Care Management Education program

The Care Management Education program is a 100 percent online certificate program designed for nurses or other health professionals currently working in a care manager capacity, or aspiring to enter this growing field.

This program is designed to help you gain and refine the essential knowledge and skills necessary to work in a doctor’s office, hospital, skilled nursing facility, home health care and other settings. 

Why Take This Program?

The program consists of 10 modules, each covering an essential aspect of the care manager role. To complete the program, take one module at a time, or all 10. Those who complete all 10 modules will receive an Advanced Certificate of Completion in Care Management.

The series of modules provides nearly 70 continuing education credits (CNE, SW), which may also be applied to maintaining care manager national certification.

In each module, there are interactive lectures, video demonstrations, interviews with experts, reading assignments, and learning activities. Once you feel you have learned the material, you will take a short quiz to assess your learning before receiving the appropriate certificate.

Varies according to each student - you set your own pace. The program is designed to take about 68 hours for all 10 modules.

Modules

After completing this module, learners will be able to describe the factors shaping care management, including the Institute for Healthcare Improvement’s triple aim and the Affordable Care Act. Learners will be also able to explain benefits and steps to implement value based care in clinical practice.

After completing this module, learners will be able to describe models of successful care management strategies including empanelment and risk stratification. Learners will be able to explain the effective use of data warehousing and dashboard tools, and the use of data for continuous quality improvement.

After completing this module, the learner will be able to identify and describe strategies for care management, including Self-Determination Theory, motivational interviewing, and helping patients with self-management.

After completing this module, learners will be able to describe major social determinants of health and how they impact health outcomes. Learners will also be able to describe how an awareness of health literacy and development of cultural humility can improve health outcomes in diverse communities.

After completing this module, learners will be able to describe the nurse’s enhanced role on the primary care team as they collaborate to manage complex chronic illness. They will also be able to describe evidenced based practices for patient education, including the selection of appropriate educational materials appropriate to a range of health literacy levels.

After completing this module, learner will be able to describe principles of best practice for interprofessional teamwork, communication, and leadership and discuss how the TeamSTEPPS framework can support development of the skills needed to perform successfully in interprofessional teams.
