Description:

Thanks to advances in medical care, adult patients with complex chronic disease originating in the childhood now outnumber pediatric patients and we expect this trend will continue. The Center for Adults with Special Health Care Needs responds to the vital need for a system change to accommodate this growing, at-risk population. Through its core components—development of a medical home hub; enhanced care management and care coordination; workforce acquisition/development; and create of a “on-touch” resource and referral center for providers, families, and community members—the Center will provide a new, integrated model that better meets the needs of this vulnerable patient group.
