Description:

This is a study investigating the implementation of web-based client-centered care coordination to support HIV prevention, including the use of HIV pre-exposure prophylaxis, among Black men who have sex with men. The care coordination model is being implemented in selected cities across the State of New York (Brooklyn, Buffalo, Queens and Rochester). The University of Rochester will serve as a coordinating center for the four cities, in collaboration with SUNY Albany.
