Overview
The Care Management Education program is designed for nurses or other health professionals working in a care manager capacity, or aspiring to enter this growing field. Learn about the care management role over a series of four online modules designed by University of Rochester faculty:
- The Business and History of Care Management: Discover the history of the care manager role and how it has evolved to meet the changing needs of patients and communities
- Care Manager Priorities: The Nuts and Bolts of Care Management: Explore how care managers support patients through transitions in care, manage chronic and complex conditions, and address social determinants of health
- How Do We Deliver Care? Optimizing Person and Family-Centered Care: Learn how care managers drive meaningful change through person- and family-centered care, motivational interviewing, and team-based approaches
- Putting It All Together: How Lifelong Learning and Reflective Practice Makes Us Better Care Manager: Explore how care managers strengthen their skills and elevate their impact with national certification, ethical practice, and professional boundaries
It's a very dynamic, independent, and creative role.
